How it's calculated

The standard OSRS combat-level formula, unchanged since the Evolution of Combat update:

base = 0.25 × (Defence + Hitpoints + floor(Prayer / 2))
melee = 0.325 × (Attack + Strength)
ranged = 0.325 × floor(Ranged × 1.5)
magic = 0.325 × floor(Magic × 1.5)
combat level = floor(base + max(melee, ranged, magic))

What's the combat level formula?

combat level = floor(base + max(melee, ranged, magic)), where base = 0.25 x (Defence + Hitpoints + floor(Prayer / 2)), melee = 0.325 x (Attack + Strength), ranged = 0.325 x floor(Ranged x 1.5), and magic = 0.325 x floor(Magic x 1.5). Only the highest of the three combat-style components counts -- the other two don't add anything on top.

Why doesn't raising Ranged change my combat level if I'm a melee build?

Combat level only counts the single highest of your melee, ranged and magic components -- it isn't a sum of all three. If your melee component is already well ahead, Ranged (or Magic) needs to climb past it before it affects your combat level at all. The calculator's "levels to your next combat level" section shows exactly how far each stat is from making a difference.

Does Hitpoints or Prayer affect combat level as much as Attack or Strength?

No -- Hitpoints and Prayer feed the base component at a 0.25 weight (Prayer at half its level, rounded down, on top of that), while Attack and Strength feed the melee component at a higher 0.325 weight each. A point of Strength moves your combat level roughly 30% faster than a point of Hitpoints, all else equal.

What's a "virtual" combat level above what's shown in-game?

This calculator reports the real combat level the game itself uses (capped by the 99-level skill cap on every input). It doesn't extrapolate a virtual combat level past what levels above 99 (achievable, since XP keeps accumulating past the level-99 XP requirement) would produce -- the in-game combat level display already stops changing once every relevant skill is maxed.
